About The Program
City of Glendale CDBG-CV “Pandemic Response Small Business Assistance Program” a partnership of the City of Glendale and Prestamos CDFI
Grant amount: Up to $25,000 available on a first-come first-served basis.
Who Qualifies: For-Profit microenterprise business owners with 5 or fewer employees including independent artists, musicians, women, and minorities located within the City limits of Glendale Arizona.
What Can Grant be Used for: Working capital including Payroll, Lease Payment, Business loan payment, Utilities, Inventory
This program also includes access to high-quality, tactical webinars and interactive workshops in English and Spanish that provide practical tips that small businesses can immediately put into play to improve their economic position, recover from business interruptions/closures, and mitigate the hardships facing them because of the COVID-19 Pandemic.
Eligibility
- If the business is involved in one of the following activities, it is ineligible to receive funds:
- Residential landlord/developer
- Franchise
- Hotel/motel
- Liquor Store
- Chain store
- Financial/ Lending institution
- Pvt Club, fraternity/sorority organization
- Adult oriented business or sexually prurient business
- The business must be a for-profit business.
- Number of Employees must be less than 5 employees.
- The business must be located within the City of Glendale limits.
Information Collected
- Name of Business
- Legal Structure of the business
- Ownership of the business
- EIN, if any
- Date the business was established. The date must be prior to February 10, 2020.
- Business must be in good standing with the Secretary of State of Arizona
- The business will certify that if it has not received any funding from the City of Glendale AZ in the last 15 years, it has not defaulted on the payments?
- Services the business provides
- Number of Employees
- Pre-February 10, 2020
- Number of jobs to be created post-funding
- Number of jobs to be retained post-funding
- How the business will continue providing its services
- How did the business has adapted to survive COVID-19
- Amount of COVID-19 related funding the business received and from whom
An applicant who has received assistance from other CARES programs may still be eligible if such assistance is not received during the same time frame for the same purpose (no Double Dipping)
Documents Required
- Copy of business license
- Articles of Organization/Incorporation
- Cash based monthly income statement for 2019 and 2020 (year to date)
- Business tax return: 2019
- Completed W9 form for the business
- 12/31/2019 and 3/31/2020, if applicable
- Invoices/statements/bills that need to be paid with the funds
- Copy of State issued ID for all owners
